Completing this exemption application form does not constitute approval. Players must undergo an on court assessment by our Supervisors. For existing players, the ideal time frame for assessment would be during one of the last rounds of the previous season, before the new season begins in which the exemption will apply. New players can start in their desired team and age group; they will be assessed on court during the opening rounds of the season. Please read and understand Bylaws 6.4, 6.5, and 6.6 prior to lodging this request.
